If you are ready to get things rolling, please fill out the following information so I can begin to plan your ceremony.

    Let's start by getting some essential information about one of you. If one of you is the preferred contact in terms of planning, let's start with you.
    You have only three options here: "bride", "groom", or "partner". Where a man is marrying a woman, he should choose "groom" and she, "bride". In most other cases, you should both choose "partner".
  • This must match your legal name as per your birth certificate or passport.
    "X" refers to indeterminate / intersex / unspecified - please use this option if the other options don't apply to you.
  • Your regular source of employment ie: "doctor", "student", "dolphin trainer", "flower arranger", etc.
  • Usual place of residence
  • Your preferred contact number
  • (If Applicable)
    Have you been previously married? If not, choose the first option (Never Validly Married). If previous marriages are not disclosed, by law your marriage won't be deemed valid.
  • This must match your birth certificate or passport to be valid.
  • Date Format: DD slash MM slash YYYY
    As per your passport or birth certificate
  • If party born outside Australia, total period of residence in Australia (Years, months ie "10 years, 2 months"). If born in Australia, simply write "N/A".
  • Include given name, middle names and surname. If not known write "unknown". If deceased, add "deceased" after the name)

  • Who will sign the registry and legal documents? (Must appear over the age of 18)
